What the Studies Measured On How Long Kratom Lasts

Two kinds of study answer how long kratom lasts in the body, and they measure different things.

  •  Pharmacokinetic studies (aka the science of how a substance moves through the body) track blood levels. 
  • Field studies ask people how they feel, in the moment, during everyday use.

Onset: from swallow to first effects

Alkaloids reach the blood fast.

In a controlled study, every measured kratom alkaloid was detectable in participants' blood within 15 minutes of drinking kratom tea [3]. That study had six participants, so the exact figure may not hold for everyone.

There's a mechanism behind the speed, too. Anything you swallow has to be dissolved before your gut absorbs it, and a tea is already dissolved, so it skips that step [7].

Worth knowing: "detectable in blood" is the earliest possible starting line, not a promise of when you'll feel something. 

Peak: when blood levels are highest

Most studies put the peak at around one to two hours.

In a randomized, placebo-controlled trial of encapsulated leaf powder, median mitragynine levels peaked 1.0 to 1.3 hours after a single serving [2]. A similar trial of encapsulated extract found a median peak at 1.3 hours, stretching to 1.7 to 2 hours with daily use [5]. Tea peaked at about one hour [3].

What Else Changes the Clock?

Even for a single product, the answer isn't fixed. The following factors may shift the timing for how long kratom lasts in the body, and some are worth checking before consuming kratom.

Food and stomach contents

A full stomach may slow the start.

Food, especially fatty food, slows how quickly the stomach empties and how fast substances are absorbed [7]. This is general pharmacology; it has not been measured for kratom specifically.

Examples include foods relatively high in:

  • Saturated fats: butter, cheese, cream, fatty cuts of meat.
  • Mixed-fat meals: pizza, fried foods, burgers, creamy pasta, etc.
  • Unsaturated fats: avocado, nuts, seeds, olive oil.

Metabolism and liver enzymes

The liver sets part of the pace.

Lab studies using human liver enzymes found that mitragynine is broken down mainly by an enzyme called CYP3A4, which also converts some of it into 7-OH [8]. These are test-tube (in vitro) findings, which means they show the route the body uses, not how much timing differs from one person to the next.

Frequency and accumulation

Because mitragynine clears slowly, it builds up with repeated or daily use. In a controlled trial, blood levels reached a steady state, the point where intake and clearance balance out, after 8 to 9 days of daily use, and half-life lengthened as well [2]. Keep in mind: day one and day ten of consumption are not the same and do not technically start from the same baseline, even with the same product. Effects may feel different and how long kratom lasts in the body can differ. 

Medications and other substances

If you take any medication, we strongly recommend talking with your doctor or pharmacist before consuming kratom.

Kratom may change how the body processes some medications.

In a clinical study, a single serving of kratom tea raised blood levels of midazolam, a medication processed through CYP3A, by 40 to 50 percent [9]. The researchers concluded that combining kratom with certain drugs extensively metabolized by CYP3A may lead to serious interactions [9]. 

Reading a Label with Timing in Mind

A label tells you more about timing than a strain name does.

A label won't print an answer that tells you how long kratom lasts in the body, but it does hold the clues. Think of it like reading a wine label, minus the tasting notes about tobacco or leather.

  • Format. Liquid, powder, capsule or tablet sets the first step your body takes [7].
  • Leaf or extract. Extracts are concentrated. An encapsulated extract produced higher peak blood levels than earlier leaf-powder data, though that comparison spans two separate studies [5].
  • Alkaloid content. A listed mitragynine or 7-OH content tells you more than a vein color (which lab testing hasn't linked vein color to different alkaloid profiles) [4].
  • Batch number and lab report. It's a good habit to review the manufacturer's third-party lab report; many brands now add QR codes to their products that links to it. The FDA has also warned the public when certain kratom products were contaminated with Salmonella and/or concerning levels of heavy metals [1].
